OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to struck-by and crushing hazards from the Hydraulic lifts not being inspected and maintained properly:  a) Shop, Challenger Lift - Model 120007, Serial: 120277 : On or about 6/16/25, employees were operating a Challenger Lift - Model12000, Serial: 120277 that had water getting into the air hose of the lock safety mechanism which may result in the malfunction of the lift, exposing employees to crushing hazards, and the lift had not been inspected.   b) Shop, Rotary Lift - Model SPO20N0T0, Serial: DPZ23C0035: On or about 6/16/2025, employees were operating a Rotary Lift - Model SPO20N0T0, Serial: DPZ23C0035 that had a noisy power unit and had not been inspected.   c) Shop, Rotary Lift - Model SPOA10N8T0, Serial: DKR21F0266 : On or about 6/16/25, employees were operating a Rotary Lift - Model SPOA10N8T0, Serial: DKR21F0266 that had not been inspected.   d) Shop, Mohawk - Model SYSTEM IA, Serial: 9407981: On or about 6/16/25, employees were operating a Mohawk - Model SYSTEM IA, Serial: 9407981 that had not been inspected.   e) Shop, Rotary Lift - Model SPOA10N8T0, Serial: DKR21K0262: On or about 6/16/25, employees were operating a Rotary Lift - Model SPOA10N8T0, Serial: DKR21K0262 that had mis-installed lock cable and hoses and had not been inspected.   f) Shop, Rotary Lift - Model SPOA10N700, Serial: CRZ05K0367:  On or about 6/16/25, employees were operating a Rotary Lift - Model SPOA10N700, Serial: CRZ05K0367 that had a missing arm stop bolt, damaged lock actuator, mis-installed lock label and hoses, and had not been inspected.   g) Shop, Mohawk - Model SYSTEM IA, Serial: 99N: On or about 6/16/25, employees were operating a Mohawk SYSTEM IA, Serial: 99N that had not been inspected.

29 CFR  1910.1200(g)(8):  The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  a) Worksite, Service Department, Rear Bay: On or about 6/16/25, employees are required to use chemicals, such as but not limited to Johnsen's Brake Cleaner, and do not have access to the safety data sheet for the chemical product being applied. The employee was exposed to chemical hazards such as but not limited to carcinogens.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)Worksite, Service Department: On or about 6/16/25, employees are required to use chemicals, such as but not limited to Johnsen's Brake Cleaner, and were not provided information or training on the hazards associated with the chemicals.
