29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  Establishment: On or about May 3, 2025 the employer did not report the work-related hospitalization to OSHA within 24 hours but reported the hospitalization June 21, 2025. Petco Animal Supplies Stores, Incorporated was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number 1507907, citation number 1, item number 1 and was affirmed as a final order on May 21, 2021, with respect to a workplace located at 903-A Georgia Highway 124 in Braselton Georgia.
