29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) On or about June 24, 2025, at the jobsite: an employee was exposed to a 20 foot fall hazard while installing new fiber optic line  using an Altec vehicle mounted boom lift (Model AT30-G Serial 0904DC0573), in that, fall protection was not used.

29 CFR 1926.453(b)(2)(viii): An aerial lift truck was moved when the boom was elevated in a working position with men in the basket and was not specifically designed for this type of operation in accordance with the provisions of paragraphs (a)(1) and (2) of this section.  a) a) On or about June 24, 2025, at the jobsite: an employee was exposed to a 20 foot fall hazard, in that, an Altec vehicle mounted boom lift (Model AT30-G Serial 0904DC0573) was moved when the boom was elevated in a working position and a man in the basket.
