Workshop - On or about July 1, 2025, the employer failed to provide effective guarding to protect employees working at or near the JET Industrial Drill Press Machine, Model No. IDP-22, Serial No. 2209B1681, thereby exposing employees to ingoing nip points, rotating parts, and/or amputation hazards.

Wash Bay - On or around July 1, 2025, a 48-inch industrial drum fan obstructed an exit route in the wash bay, potentially hindering employee egress in the event of an emergency.  The employer failed to ensure the exit routes were kept free and unobstructed.

29 CFR  1910.178(l)(6):The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  a.) Workshop - On or about July 1, 2025, an employee used the Toyota forklift, Model 8FDU25, Serial No. 61920 to retrieve parts in the workshop.   The employer did not certify, including the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation, that each powered industrial truck operator was trained and evaluated as required by paragraph (l).
