(a) Worksite - 3rd Floor: On or about 7/14/2025, an employee was allowed to ride on a scaffold when the conditions listed in 29 CFR 1926.452(w)(6)(i) - (v) did not exist; Outrigger frames, when used, were not installed on both sides of the scaffold.
