29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about July 15, 2025, during the installation of roofing material at a job site located at 11611 Lincoln Way East, Orrville Ohio 44667; the employer did not ensure employees working on a roof of a commercial location were protected from a fall of approximately 17.2 feet by utilizing a fall protection system.   The  TRADITIONAL ROOFING, LTD DBA TRADITIONAL ROOFING LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1520433, citation number 1, item number 1a and was affirmed as a final order on 5/14/2021, with respect to a workplace located at Next to 9493 Emerald Isle ST NE, Massillon Ohio 44646.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about July 15, 2025, employees were exposed to a fall of approximately 17.2 feet while performing roofing activities on a commercial roof located at 11611 Lincoln Way East, Orrville Ohio 44667.  The employees were using a portable ladder to access/egress the working surface of the roof that did not extended three feet above the landing area.   The  TRADITIONAL ROOFING, LTD DBA TRADITIONAL ROOFING LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) Ladders), which was contained in OSHA inspection number 1520433, citation number 1, item number 2 and was affirmed as a final order on 5/14/2021, with respect to a workplace located at Next to 9493 Emerald Isle ST NE, Massillon Ohio 44646.
