This exposed employees to fall hazards causing serious injury and/or death not only from the fall itself, though because the body belt could cause injury in itself. At a new residential worksite located at 10415 N Liberty St, KC MO -- employees were using an extension ladder to gain access to the roof. The ladder did not extend 3 feet above the upper landing surface (roof). This exposed employees to fall hazards greater than 11' to ground level. This was documented on or about 7/18/2025.
