The employer must ensure: 29 CFR 1910.23(b)(13): No employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder. a. Located at 2403 Pennsylvania Avenue, Wellston, Ohio, 45692: On or about July 15, 2025, the employer did not ensure that material was not carried by employees while climbing a scaffolding ladder which could cause a loss of balance and fall while climbing up or down the ladder. On or about July 15, 2025, an employee was hospitalized as a result of injuries sustained from falling from a scaffolding ladder while carrying 18 inch x 18 inch sheets of plastic up the scaffolding ladder.

Located at 2403 Pennsylvania Avenue, Wellston, Ohio, 45692: On or about July 15, 2025, and times prior, the employer failed to ensure a competent person inspected the 3 tier scaffolding system for visible defects before each shift.
