29 CFR  1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  a) On or about July 25, 2025, the employer did not ensure that personal protective equipment for eyes, face, and hands was provided for employees working with soluble oil, motor oil, coolant, and degreaser. Employees were exposed to health hazards related to eyes and skin contact when transferring, mixing, and diluting hazardous chemicals for use in running normal production on industrial machines.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about July 25, 2025, the employer did not ensure adequate safeguarding of the point-of-operation and its associated danger zone on the Okuma Cadet V4020 CNC machine. Employees operating this CNC machine were exposed to struck-by and caught-in hazards of moving parts during normal production because safety interlock device was removed from the sliding access door allowing it to run with the door opened.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:  On or about July 25, 2025, the employer did not ensure that examination of flexible cords for handheld portable angle grinder and drill was conducted to see damaged insulation exposing energized internal wiring. 29 CFR  1910.179(j)(2)(iii): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: Hooks with deformation or cracks. Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected. For hooks with cracks or having more than 15 percent in excess of normal throat opening or more than 10�� twist from the plane of the unbent hook refer to paragraph (l)(3)(iii)(a) of this section.  On or about July 25, 2025, the employer did not ensure that a daily visual inspection of the overhead jib crane's hook was conducted to determine a damaged safety latch. Employees were  exposed to hazards associated with lifting and handling a load on a hook with a damaged safety latch.
