An employee was wearing a body belt that had the lanyard attachment point located at the front of the workers waist while conducting residential roofing operations.

The employees were not trained in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.
