29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: A): 15 Lincoln Avenue, Tuckahoe - NY: Employees were exposed to fall hazards of approximately 15-feet while using an extension ladder to access a roof. The extension ladder that was used, was only extended approximately 2-feet above the landing and not the required 3-feet, on or about, July 29th, 2025.
