29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:   On or about 07/31/2025, at 1340 W. 41st Street, Hialeah, Florida, employees were exposed to an electrical shock hazard while working with energized electrical wiring to reconnect the wiring of an HVAC unit, in that the employer did not ensure the circuit was deenergized nor that electrical protective equipment such as rubber insulating gloves were provided and used to insulate the employees before employees performed the work.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:  On or about 07/31/2025, at 1340 W. 41st Street, Hialeah, Florida, employees were exposed to an electrical shock hazard while working with energized electrical wiring to reconnect the wiring of an HVAC unit, in that the employer did not provide and ensure employees used electrical protective equipment such as rubber insulating gloves nor that the circuit was deenergized before employees performed the work.

29 CFR 1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take:  On or about 07/31/2025, at 1340 W. 41st Street, Hialeah, Florida, employees were exposed to electric shock hazards while working with an HVAC unit, in that the employer did not ensure that proper warning signs were posted and employees were advised of the hazards involved and the protective measures to be taken before employees performed the work.
