29 CFR 1910.101(b):"Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. Before returning empty cylinders, close the valve and see that cylinder valve protective caps and outlet caps or plugs, if used, are replaced. Cover label with Empty Label meeting ICC requirements, or if cylinder is provided with combination shipping and caution tag remove lower portion. A) In the casino's maintenance shop on (or about) August 12, 2025, employees were exposed to struck-by hazards in that two compressed gas cylinders were found to be stored in the shop area near the table saw, with the protection caps not installed over the valves.

29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. A) In the maintenance shop, on or about August 12th, 2025, employees performing maintenance activities utilizing two drill presses (SN: 17103677, and SN: AZ053 76125) that were missing a front guard to protect the operator from the rotating drill bit, and chuck were exposed to caught-in hazards.

29 CFR 1910.213(c)(1):Each circular hand-fed ripsaw was not guarded by a hood which shall completely enclose that portion of the saw above the table and that portion of the saw above the material being cut. The hood and mounting shall be arranged so that the hood will automatically adjust itself to the thickness of and remain in contact with the material being cut but it shall not offer any considerable resistance to insertion of material to saw or to passage of the material being sawed. The hood shall be made of adequate strength to resist blows and strains incidental to reasonable operation, adjusting, and handling, and shall be so designed as to protect the operator from flying splinters and broken saw teeth. It shall be made of material that is soft enough so that it will be unlikely to cause tooth breakage. The hood shall be so mounted as to insure that its operation will be positive, reliable, and in true alignment with the saw; and the mounting shall be adequate in strength to resist any reasonable side thrust or other force tending to throw it out of line. A) In the maintenance shop, on or about August 12th, 2025 the table saw that was located in the maintenance shop did not have a hood guard that covered the saw blade. Employees using the saw are directly exposed to the portion of the blade that was above the material being cut and therefore amputation hazards.

29 CFR 1910.213(c)(2):Each hand-fed circular ripsaw shall be furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator. The spreader shall be made of hard tempered steel, or its equivalent, and shall be thinner than the saw kerf. It shall be of sufficient width to provide adequate stiffness or rigidity to resist any reasonable side thrust or blow tending to bend or throw it out of position. The spreader shall be attached so that it will remain in true alignment with the saw even when either the saw or table is tilted. The provision of a spreader in connection with grooving, dadoing, or rabbeting is not required. On the completion of such operations, the spreader shall be immediately replaced. A) In the casino maintenance shop, on or about August 12, 2025, employees performing maintenance activities with a hand fed ripsaw that was missing a spreader to keep the material being cut from catching the saw blade and kicking the material back at the operator exposing them to struck-by hazards.

They shall be designed to provide adequate holding power for all the thicknesses of materials being cut. A) In the casino maintenance shop, on or about August 12, 2025, employees performing maintenance activities utilizing a table saw that was missing kickback fingers to prevent the material being cut from striking the operator in case the material caught the blade and kicked back into the operator.
