29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly:   a) On or about August 20, 2025, at a workplace located at 190 North Katch Drive, Oconto, Wisconsin; the employer did not ensure that the Yale GLP050VXNVSE084 LP forklift nameplate reflected the accurate length of the forks installed on the forklift prior to use. The forks installed on the lift were 59.75 inches long when the nameplate indicated the forks were 42 inches long, exposing employees to struck-by hazards.   b) On or about August 20, 2025, at a workplace located at 190 North Katch Drive, Oconto, Wisconsin; the employer did not ensure that the Yale GLP050VXNVSE084 LP forklift nameplate reflected the use of 8 ft. fork extensions used on the forklift prior to use. The nameplate indicated the forks were 42 inches long, exposing employees to struck-by hazards.   c) On or about August 20, 2025, at a workplace located at 190 North Katch Drive, Oconto, Wisconsin; the Yale GLP050VXNVSE084 LP forklift had a hole removed from the fork tip without the manufacturer's prior approval. The modification to the fork would affect the capacity and safe operation exposing employees to struck by hazards.   d) On or about August 20, 2025, at a workplace located at 190 North Katch Drive, Oconto, Wisconsin; the employer did not ensure that the Yale GLP050NNNXALP forklift nameplate reflected the accurate length of the forks installed on the forks prior to use. The forks installed on the lift were 59.875 inches long when the nameplate indicated the forks were 42 inches long, exposing employees to struck-by hazards.
