29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a) Work site - On or about August 19, 2025 and at times prior thereto, employees were using a fabricated frame scaffold that had not been inspected by a competent person for visible defects, as it had obvious defects in that no guardrails were installed on the top of the second tier.

29 CFR  1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound top rail capacity):   a) Work site - On or about August 19, 2025 and at times prior thereto, employees were working from the second level of a scaffold at approximately 14 feet above the ground with no guardrails or personal fall arrest system in place.

29 CFR  1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects:    a) Work site - On or about August 19, 2025 and at times prior thereto, employees were working from the second level of a scaffold approximately 14 feet above the ground without toeboards while employees were working directly below on the ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Work site - On or about August 19, 2025 and at times prior thereto, employees were accessing the second level of a scaffold from an extension ladder that extended approximately 6 inches above the  landing surface and was not secured to the scaffold.
