29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at it's top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 112 Josephs Lane, Pittsburgh, PA: On or about August 21, 2025 during deck installation activities, the employer did not ensure a ladder, used to get down from the decking, was set up properly in that it was configured to be used as a stepladder and was positioned below the decking and joist level, exposing an employee to a fall hazard of approximately 15 feet.

29 CFR  1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person designated by the employer:   a) Job site, 112 Josephs Lane, Pittsburgh, PA: On or about August 21, 2025 during deck installation activities, the employer did not ensure a system of hazard identification and abatement was implemented at that job site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection, using a ladder in a safe manner, using nail guns without proper eye/face protection, climbing on the guardrails of lift units, leaving a forklift raised in the air, rebar at the site without caps, depressions in the ground not being covered or barricaded & tripping hazards being present.
