The employer did not ensure that the Pro Tector needle sheath safety device was used to eliminate or minimize employees exposure to bloodborne pathogens.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19

On or about August 14, 2025 - William F. Ryan Community Health Center, Inc . located at 565 Manhattan Ave, New York, NY 10027  a) The employer did not document consideration and implementation of safer needles devices such as but not limited to the Pro Tector disposable needle sheath prop in their annual review of their exposure control plan.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
