Location: Job site;  On or about 08/27/2025, employees were allowed to utilize Alum-A-Pole pump jack scaffolding which was not erected by experienced employees under the supervision of a competent person qualified in scaffolding erection. The deficiencies with the pump jack scaffolding units which were not correctly erected or whose deficiencies were not identified and corrected prior to usage by a competent person are noted in the subsequent violations in 1-2-b through 1-2-f.
