29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about September 02, 2025, during a construction project on the roof of a residential building located at Butternut Ridge Apartments, 5620 Great Northern Blvd, North Olmstead, Ohio 44070. The employer did not ensure that employee(s) were protected from possible danger of eye injuries from impact and/or flying debris by operating pneumatic nail guns without the use of eye protection.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about September 02, 2025, during a construction project on the roof of a residential building located at Butternut Ridge Apartments, 5620 Great Northern Blvd, North Olmstead, Ohio 44070, the employer did not ensure that employee(s) were protected from fall hazards. Employees were observed performing roofing operations without the use of fall protection. The building had multiple roof heights. A single-story garage roof on the North West side of the building was measured at 9 feet, 2 inches, roof eave to ground at an approximate 1/12 pitch. The main part of the building, the South West side, was two stories, measured at 18 feet, 4 inches, from the roof eave to the ground with an approximate 6/12 pitch.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    On or about September 02, 2025, during a construction project on the roof of a residential building located at Butternut Ridge Apartments, 5620 Great Northern Blvd, North Olmstead, Ohio 44070, the employer did not ensure  that the ladder on the portable ladder used by employee(s) to gain access to the roof were extended 3 feet above the landing surface.
