29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about September 26, 2025, during a construction project on the roof of a residential building located at Butternut Ridge Apartments, 5770 Great Northern Blvd, North Olmstead, Ohio 44070, the employer did not ensure that employee(s) were protected from fall hazards. Employees were observed performing roofing operations without the use of fall protection. The building had multiple roof heights. A single-story garage roof on the North side measuring approximately 9 feet, 6 inches, roof eave to ground at an approximate 3/12 pitch. The main part of the building, also the North side, was two stories, measured at 19 feet, 9 inches, from the roof eave to the ground with an approximate 6/12 pitch.
