29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  (a)	On or before September 4, 2025, the employer did not ensure that each employee that worked in a trench at the downhill end of the project was provided with a safe means of egress, in that the ladder used to access the trench was located upslope approximately 50 feet away from where employees handled water pump hoses that were in the process of removing water. Employees were exposed to cave-in hazards due to the trench being approximately 10 feet deep and having near vertical walls due to soil sloughing.   (b)	On or before September 4, 2025, the employer did not ensure that each employee that worked inside a trench box shield system that was installed near the middle of the project was provided with a safe means of egress, in that the ladder used to access the trench was located upslope and approximately 15 feet away from the end of the trench box.  Employees were exposed to cave-in hazards due to the trench being approximately 10 feet deep and having near vertical walls.

29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation:  (a)	On or before September 4, 2025, the employer did not protect employees from cave-in hazards when they worked inside the trench at the downhill end of the project, in that employees worked outside of a trench box shield system when they handled water pump hoses that were in the process of removing water at the bottom of the trench. The trench was approximately 10 feet deep and had nearly vertical walls due to soil sloughing.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):  (a)	On or before September 4, 2025, the employer did not ensure that employees who worked inside the trench at the downhill end of the project were protected from cave-in hazards in that employees at this location worked outside of a trench box shield system. Employees handled water pump hoses that were in the process of removing water at the bottom of the trench.  The trench was approximately 10 feet deep and had nearly vertical walls due to soil sloughing.   (b)	On or before September 4, 2025, the employer did not protect employees from cave-in hazards when they worked inside the trench at a location that was upslope and outside of a trench box shield system to tie in a water line.  The trench was approximately 10 feet deep and the soil walls were nearly vertical.
