29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about September 5, 2025, it was observed that employee(s) engaged in commercial roofing operations located at S. 71st Ave and Beaufort Drive, Papillion, Nebraska. Employee(s) were exposed to fall hazards in that the employer had employee(s) working at heights greater than 14 feet without any form of fall protection in place.

29 CFR  1926.1053(b)(1): The employer is exposing employees to fall hazards where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about September 5, 2025, employee(s) were engaged in commercial roofing activities at S. 71st Ave and Beaufort Drive. Papillion, Nebraska. Employee(s) were using a ladder to access and egress the working surface exposed to fall hazards. The ladder was not extended at least 3 feet over the upper landing surface.
