29 CFR  1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   Location: 1991 Oakland Ave, Wantagh, NY 11793  On or about 09/16/2025, employee working from a pump jack scaffold was not protected from falling approximately 21ft while installing new windows on a two story residential home.   Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  Location: 1991 Oakland Ave, Wantagh, NY 11793  On or about 09/16/2025, an employee was installing windows on a residential home and used an extension ladder to access the upper landing. The ladder was only extended 1ft above landing.  Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

Location: 1991 Oakland Ave, Wantagh, NY 11793  On or about 09/16/2025, an employee used an unsecured extension ladder set up on a pitched roof to access the roof while installing windows.    Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
