29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   On or about September 25, 2025, employees performing roofing operations were exposed to fall hazards and eye injuries, and the employer did not ensure a competent person inspected the jobsite for hazards and take necessary steps to eliminate and control them:  a. Employees working on the roof were exposed to fall hazards greater than 10 feet and fall protection was not used.  b. Employees using pneumatic nail guns were exposed to eye injuries and safety glasses were not used.  c. Employees working on the roof were exposed to fall hazards from open holes in the roof decking and the holes were not effectively covered or guarded.  d. Employees were exposed to fall hazards while using an extension ladder and the side rails did not extend at least three feet above the roofs edge.  e. Employees were exposed to fall hazards greater than 10 feet while working from the raised truck bed and fall protection was not used.
