29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a. On or about October 6, 2025, at 1459 Oconner Ave Melbourne, FL: employees were exposed to 10 foot 1 inch fall hazards while re-roofing a 6:12 pitched roof, in that a fall protection system was not used.  Poe Roofing & Consulting Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1712706, citation number 1, item number 2 and was affirmed as a final order on March 06, 2024, with respect to a workplace located at 3939 W Gandy BLVD Tampa FL 33611.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about October 6, 2025, at 1459 Oconner Ave Melbourne, FL: : employees were exposed to 10-feet 7-inch fall hazard, in that an extension ladder was used and did not extend 3-feet above the roof landing to access a 6:12 pitch residential roof.  Poe Roofing & Consulting Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1712706, citation number 1, item number 1 and was affirmed as a final order on March 06, 2024, with respect to a workplace located at 3939 W Gandy BLVD Tampa FL 33611.
