Material stored in tiers was not stacked, blocked, interlocked, or limited in height so that it was stable and secure against sliding and collapse:  On or about October 20, 2025, concrete burial boxes in the storage area were stacked on top of each other while resting on uneven ground surfaces and inconsistently sized cribbing such as weathered nominal lumber, creating instability in the stack.

(b) A defective two-legged bridle wire rope sling configured as two basket hitches was being used.
