On or about October 10, 2025, at 2662 Indian River Pkwy, Mims, FL: employees were exposed to a 10 foot fall hazard while conducting framing activities on a 4:12 pitched roof, in that, employees used an extension ladder to gain access to the roof and the siderails extend approximately 18 inches above the roof landing.
