29 CFR 1910.30(b)(1): The employer did not train each employee on or before May 17, 2017 in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment:  a) Worksite, Cafeteria, Gorilla Ladder: On or about 10/21/25, an employee was using a Gorilla GLMPXA-22 Aluminum Telescoping Multi-Position Ladder to clean windows approximately 13 feet above the floor, and the employer did not train the employee in the proper use of the ladder.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident:  a) Worksite - Cafeteria:  On or about October 22, 2025, the employer failed to report a hospitalization as a result of a work related incident when an employee fell of a Gorilla ladder while cleaning windows which occurred on October 21, 2025 to OSHA within 24 hours after learning about it.
