OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to crush-by hazards:  a) Establishment, Repair bay 3: On or about November 19, 2025, employees were operating a automotive Hydraulic lift, Blazer 9000 Lube lift, where the safety structures left and right were not engaging when lifted. Employees were exposed to crush by hazards by not having the safety structures working properly.
