29 CFR  1910.147(c)(7)(i)(C): Employees whose work operations are in an area where energy control procedures are utilized, shall be instructed about the procedure:  On or about 11/18/2025, employees were exposed to caught-in-between hazards while working at a manufacturing facility located at 3850 Millstone Parkway in Saint Charles, Missouri. An employee was removing debris from the belt of an incline conveyor when a second employee engaged the machine allowing the employee to enter the danger zone with live parts in operation.

29 CFR  1910.147(d)(2):The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:  On or about 11/18/2025, employees were exposed to caught-in-between hazards while working at a manufacturing facility located at 3850 Millstone Parkway in Saint Charles, Missouri. An employee was removing debris from the belt of an incline conveyor which did not have it's energy source properly isolated allowing the employee to enter the danger zone with live parts in operation.
