29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  a) On November 19, 2025, at the above mentioned address, an employee of Humberto Garcia dba Humberto Garcia was exposed to eye injury while operating a leaf blower to remove roofing debris without eye protection. 29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) On or about November 19, 2025, at the above-mentioned address, employees were exposed to fall hazards of approximately 18 feet ground to eave while performing residential roofing work.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a) On or about November 19, 2025, at the above mentioned address, the employees were exposed to fall hazards of approximately of 18 feet ground to eave while performing residential roofing work. Employees were connected to the fall protection lanyard at the nylon webbing at their hip.  Humberto Garcia dba Humberto Garcia was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(17) which was contained in OSHA inspection number 1749510, citation 1, item 2, and was affirmed final order on September 9, 2024, with respect to a workplace located at 1570 Britanny lane, Hoffman Estates, IL 60192.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
