29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a) On or about November 20, 2025, at jobsite location 2200 Southwest 45th St. Fort Lauderdale FL 33312, the employer did not ensure that the Toyota 6BPU15 Order picker Serial #80456 powered industrial truck with a damaged wheel was taken out of service thereby exposing employees to a struck by and/or crushed by hazards .
