29 CFR 1910.119(e)(3)(iii): The process hazard analysis did not address engineering and administrative controls applicable to the hazards and their interrelationships such as appropriate application of detection methodologies to provide early warning of releases: On or about September 26, 2025, the Cracked Absorber Unit (CAU) / Deisohexanizer (DIH) / H2S Absorbers Process Hazard Analysis (PHA), Node 14 Depropanizer, Deviation 10 Abnormal Operations, Causes 12 Sampling - Sampling error (Deprop tops) and 13 Corrosion/Erosion - Pipe or equipment leak or failure: The PHA did not address engineering and administrative controls applicable to the fire and leak hazards that were associated with the Applied Automation Sampling System located at the C3 sampling station.
