29 CFR 1910.1200(e)(2): The employer that produced, used, or stored hazardous chemicals at the workplace in such a way that employees of other employer(s) could be exposed did not ensure that the hazard communication program included all of the elements outlined in 29 CFR 1910.1200(e)(2)(i) through 29 CFR 1910.1200(e)(2)(iii).  (a) Dental Suite: On or about September 22, 2025, the employer used hazardous chemicals in the workplace including the dental fixative Formo-Creosol in such a way that employees of other employer(s) could be exposed and did not ensure that their hazard communication program included all of the elements outlined in 29 CFR 1910.1200(e)(2)(i) through 29 CFR 1910.1200(e)(2)(iii).  Abatement Note: Employers who produce, use, or store hazardous chemicals at a workplace in such a way that the employees of other employer(s) may be exposed shall ensure that the hazard communication programs include the following:  1910.1200(e)(2)(i) The methods the employer will use to provide the other employer(s) on-site access to safety data sheets for each hazardous chemical the other employer(s)' employees may be exposed to while working; 1910.1200(e)(2)(ii) The methods the employer will use to inform the other employer(s) of any precautionary measures that need to be taken to protect employees during the workplace's normal operating conditions and in foreseeable emergencies; and, 1910.1200(e)(2)(iii) The methods the employer will use to inform the other employer(s) of the labeling system used in the workplace.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  (a) Dental Suite: On or about September 22, 2025, employees were not provided effective information and training on hazardous chemicals in their work area, in that employees were not aware of the hazards and control measures necessary to protect themselves from chemicals including  Dentonics, Inc. Formo -Cresol.
