29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) 22924 Sussex Hwy., Seaford, DE 19973 (Construction Site): On or about December 9, 2025 and times prior there to, employees were exposed to fall hazards from approximately 9 feet when employees worked from frame scaffold (supported scaffold) that had been erected without base plates and/or mud seals. b) 22924 Sussex Hwy., Seaford, DE 19973 (Construction Site): On or about December 9, 2025 and times prior there to, employees were exposed to fall hazards from approximately 20 feet when employees worked from frame scaffold (supported scaffold) that had been erected without a competent person.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound top rail capacity). a) 22924 Sussex Hwy., Seaford, DE 19973 (Construction Site): On or about December 9, 2025, and times prior there to employees were exposed to fall hazards from approximately 20 feet when conducting overhand block laying activities without the use of a guardrail system, personal fall arrest system, or end guards on frame scaffold (supported scaffold).
