29 CFR 1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed. On or about December 11, 2025, and at times prior, in the employer's rail yard, employees were utilizing portable ladders to perform elevated work on railcar attachments when the employees over-reach and place their center of gravity beyond the ladder's side rails.
