29 CFR  1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 6437 North Colorado Avenue in Kansas City, Missouri: Employees were exposed to eye injury hazards.  Employees were observed using pneumatic nail guns during roofing activities without eye protection.  Employees were exposed to flying debris and projectiles.

Employees were observed performing residential roofing work while installing roofing materials on a roof without the use of fall protection.  Employees were exposed to a fall distance from the roof edge to the ground of 16 feet.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the residential construction site located at 6437 North Colorado Avenue in Kansas City, Missouri: Employees were exposed to fall hazards while accessing the roof.  Employees used a portable ladder that did not extend at least three feet above the landing surface.  The ladder did not provide proper access to the roof surface.
