29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Worksite was located at 615 Ocean Road, Spring Lake, NJ 07762  Two workers were exposed to fall hazards of approximately sixteen feet to ground from the pump jack scaffolds and gable roof they were working from as they were not utilizing fall protection. The scaffold platforms had no guardrails or safety nets and they had no fall protection on the gable. The crew told CSHO they were given no training by the employer in safety or hazard recognition.  Violation occurred on or about January 5, 2026  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.451(c)(3):Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:  a) Worksite was located at 615 Ocean Road, Spring Lake, NJ 07762  Two workers were exposed to fall hazards of approximately sixteen feet to ground from the pump jack scaffold on the east end of the front of the house they were working on as the scaffold did not have plumb posts.   Violation occurred on or about January 5, 2026  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

The scaffold platforms had no guardrails or safety nets in place while the crew was working from them. 29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Worksite was located at 615 Ocean Road, Spring Lake, NJ 07762  Two workers were exposed to fall hazards of approximately sixteen feet to ground from the gable roof they were working on as they were not utilizing fall protection.   Violation occurred on or about January 5, 2026  Huima Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1647545, citation number 1, item number 1 and was affirmed as a final order on 5/30/2023, with respect to a workplace located at 85 Seaview Avenue, Monmouth Beach, NJ 07750.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
