(a) On or about January 20, 2025, the employer did not ensure semi-trailer trucks backed into the loading dock, used wheel blocks while forklift truck operators were loading/unloading merchandise into and out of its trailer therefore exposing forklift operators to crushed-by, caught in-between, and fall hazards.
