On or about 01/22/26 the company worksite Foreman was operating a JLG 6042 All-Terrain lift to deliver and support 4 x 8 yellow sheets of Densglass firewall to himself and company workers, who were working from a residential structure's second floor.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
