On or about 01/22/26 the company worksite Foreman was operating a JLG 6042 All-Terrain lift to deliver and support a dog house dormer's pitched roof trusses to the company worksite Foreman and a company worker and performing other similar activities with this equipment.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
