29 CFR 1926.502(d)(16): 1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level a- 20 Handbell Lane, Sicklerville NJ: A worker was exposed to fall hazards when working and walking from a residential structure's exterior wall top plates when their full body harness was connected to a nylon lanyard whose length and having been anchored by being wrapped around a single, unrated, wooden, upper chord of pitched roof truss (that had not received its final bracing) would not have prevented this worker from contacting the flooring below. Condition observed on or about 02/27/26.

Condition observed on or about 02/27/26. Fortex Contractor Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1813384, citation number 1 , item number 1 and was affirmed as a final order on 06/25/2025, with respect to a workplace located at 2801 Creek Road, Hainesport, NJ. A worker, not using fall protection as well as improperly using fall protection, was exposed to fall hazards when walking and working from a residential structure's exterior wall frame top plates while performing truss acceptance, installation, securing and performing other related activities. This company worker had not received fall protection training as specified in 29 CFR 1926.503 (a)(2)(i) through (viii). Condition noted on or about 02/27/26. Fortex Contractor Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(2), which was contained in OSHA inspection number 1813384, citation number 1 , item number 2 and was affirmed as a final order on 06/25/2025, with respect to a workplace located at 2801 Creek Road, Hainesport, NJ.
