This condition exposed employees to struck-by hazards while working directly adjacent to and below roof truss framing activities.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:   (a)Guerra Framing, LLC at 9001 East Knox Circle, Sioux Falls, SD 57110: On or about and at times prior to March 4, 2026, the employer did not ensure that anchorages associated with rope grab fall protective systems were adequate and would support an employee in the event of a fall. The system was looped around the top chord of a roof truss system and tied back to itself, exposing the synthetic rope to the sharp edges with the hazard of potentially shearing under a dynamic load. This condition exposed the employees to fall hazards greater than six feet.

29 CFR  1926.502(d)(16): 1926.502(d)(16): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level:  (a)Guerra Framing, LLC at 9001 East Knox Circle, Sioux Falls, SD 57110: On or about and at times prior to March 4, 2026, the employer did not ensure that the personal fall arrest system used by employees limited the free fall distance to 6 feet, nor prevented contact with the lower level.  Employees engaged in framing activities while standing on the top plates of framed walls were wearing full body harnesses connected by a shock absorbing lanyard anchored to a roof truss systems; however, their rope grabs were not properly adjusted to limit free fall distance to six feet. This condition exposed the employees to fall hazards greater than six feet.
