29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Jobsite: On or about March 9, 2026, employees were exposed to eye injury hazards while operating pneumatic nail gun  to secure shingles without the use of required eye protection.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   On or about March 9, 2026,  employees were exposed to a fall hazard of at least 6 feet while performing roofing work on a building without providing or ensuring the use of any fall protection system. The employees were not protected by guardrails, safety nets, or personal fall arrest systems while working at elevated heights.

29 CFR  1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface:  Jobsite: On or about March 9, 2026, employees were using aluminum extension ladders to access the roof and were not protected from a fall hazard where the ladders did not extend three feet above the landing surfaces. The ladder was placed on snow-covered ground, creating an unstable base and increasing the likelihood of shifting, slipping or collapse during use.
