29 CFR  1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s)  a) On the site located at 25515 Old Landfill Rd, Port Charlotte, FL: On or about 03/13/2026, the employer exposed employees to crushing hazards, in that, cylinder sleeves used as lockout devices preventing the tailgates of trucks from falling, due to gravitation energy, did not identify the employee who applied the device.
