29 CFR  1910.178(m)(2): Employee(s) stood under or passed under the elevated portion of a powered industrial truck(s):  On or about March 18, 2026, Metal Supermarkets Cleveland (South) failed to prohibit employee(s) from standing or passing beneath an elevated 40' steel beam simultaneously supported by a Clark Forklift (Model C30L, S/N P232L-0717-9839 CNF) and a Toyota Propane Forklift (Model 8FGU25, S/N 50082) using a Forklift Jib Boom Crane attachment and/or choker straps, resulting in exposing employee(s) to struck-by hazards.

29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  On or about March 16, 2026 Metal Supermarkets Cleveland (South) did not have written approval an adjustable forklift jib boom crane (5500lb capacity) forklift attachment being used to transport 20', 40, and 50' steel beams from outside the building to inside the building.
