29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: A) 48 Taunton Road, Scarsdale - NY: Employees were exposed to fall hazards of approximately 20 - 1/2 feet while using an extension ladder to access a bracket scaffold. The extension ladder that was used, was only extended approximately 1 1/2-feet above the landing and not the recommended 3-feet as required by the manufacturers instructions, on or about, March 19th, 2026.
