29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person. Site- Employees were not protected from a fall of approximately 15 feet where the employer did not provide a qualified person to supervise the installation of anchor points for personal fall arrest equipment in use. Personal fall arrest systems for three employees working from the roof included the use of the same anchor points, the equipment used in the common anchor points were not rated for 5,000 pounds per employee; and the equipment was not protected from abrasion where life lines were wrapped around aluminum solar panel mounts.
