OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to struck-by hazards while performing cemetery groundskeeping duties.   Location: By Rows/Area marker 23, 4C, 3, 17, 13, 2, 1, 7, 4, 6 , 21  On or about March 18, 2026, Employees were exposed to struck?by hazards from overhead "widowmaker" branches-broken, dead, or unstable limbs that can fall without warning. Employees were exposed while performing cemetery groundskeeping duties such as weed and grass trimming, lawn mowing, digging burial plots and general landscaping.

29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  Location : Mausoleums   On or about March 18, 2026, employees were exposed to fall, and crushed by hazards where scissor lift scaffolds had not been inspected and removed from service due to inoperable control panels.
