29 CFR 1926.102(a)(1): The employer did not  ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) New residential construction located at Lot 750 Foxtrot Ave, Hoschton, GA 30548, Top plate: On or about 03/19/2026, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided to employees operating these tools at 130 PSI.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) New residential construction located at Lot 750 Foxtrot Ave, Hoschton, GA 30548, Top plate: On or about 03/19/2026, the employer exposed employees to fall hazards of approximately 19 feet to the lower ground level, in that a fall protection system was not provided employees engaged in framing activities.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Residential construction located at Lot 750 Foxtrot Ave, Hoschton, GA 30548: On or about 03/19/2026, the employer exposed employees to fall hazards of approximately 19 feet to the lower level. Employees engaged in framing activities had not been provided with fall protection training to recognize fall hazards and the procedures to follow to minimize those hazards.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge:  a) Residential construction located at Lot 750 Foxtrot Ave, Hoschton, GA 30548, Stairway: On or about 03/19/2026, the employer exposed employees to fall hazards of approximately 6-feet to the lower concrete surface, in that a stair rail was not installed along the open sides of the stairway used by employees to access the second level.

29 CFR  1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) New residential construction located at Lot 750 Foxtrot Ave, Hoschton, GA 30548: On or about 03/19/2026, the employer failed to inspect all electrical extension cords used to provide power to electric tools, in that an orange 50-ft. electrical cord was damaged and exposing the internal insulated energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
